This form is for individuals registered with Fitness Passport through an eligible organisation, such as:

New Zealand Police Association
Fire and Emergency New Zealand (FENZ)
New Zealand Corrections
New Zealand District Health Boards (DHBs)
Other participating employers or sponsors

If you're unsure whether you're eligible, please confirm with your HR department or Health & Wellbeing team to check if your organisation participates in the Fitness Passport Program.
To proceed, you must:

Have access to the Fitness Passport portal
Provide a valid Membership ID
